Mid-Toronto Diabetes Program
The Mid-Toronto Diabetes Program provide services and support by an interdisciplinary team of health professionals for adults with Type 2 diabetes, pre-diabetes, and for those at high risk of developing diabetes.
Services are FREE of charge
Over-the-phone language interpretation services are available as needed

Managing Your Diabetes
We integrate an accessibility positive approach to care by providing the following:
Our Certified Diabetes Educators (CDE), Registered Nurse and Registered Dietitian:
- Facilitate Monthly Virtual group education workshops
- Diabetes Prevention
- Diabetes Management
Provide individual counselling sessions over the phone and/or in person as necessary.
Our skill-building sessions help you set realistic and achievable goals to self-manage your diabetes and reduce your risk of developing possible diabetes-related complications.

- Tips and benefits of blood sugar monitoring
- Recognizing the signs and symptoms of high and low blood sugars
- Healthy eating for diabetes prevention and management
- Meal and snack planning for blood sugar control

- Reading food labels and smart shopping strategies
- Diabetes medication management
- Foot care and diabetes

Foot Care Clinic
Our Chiropody (Foot Care) Clinic provides maintenance of general foot health and managing unexpected foot ailments.
In order to assist clients in their overall health, we have a Chiropody (Foot Care) Clinic for the maintenance of general foot health and managing unexpected foot ailments. We provide assessments and treatment for the following: diabetic foot ulcers, warts, skin infections, fungal nails, heel pain, foot pain, and other foot problems. Soft-tissue surgeries are also available for a variety of reasons, such as ingrown toenails.
