Overview

Primary healthcare refers to basic, everyday health care. Primary healthcare could be visiting the doctor, nurse or nurse practitioner, talking to a dietitian, or getting education on things like diabetes. It also can involve counselling and care coordination. It is usually your first encounter with a health care provider when you need care or advice.

At Vibrant Healthcare Alliance, we believe in a client-centered, multi-disciplinary approach to primary health care. This means that our clients are at the centre of any planning and decision making about their treatment and/or care. Our team of doctors, nurse practitioners, nurses, therapists, occupational therapist, certified diabetes educators, dietitians, physiotherapist, chiropodists, and personal support workers work closely together.

They provide the best information and guidance to our clients and support them in the decisions they make about their health care. The care provided is confidential. The primary health care team encourages clients to make use of the health promotion and prevention programs offered at Vibrant Healthcare Alliance and in the community.

Doctors & Nurses

Vibrant Healthcare Alliance provides accessible primary health care services for people with physical disabilities

We integrate an accessibility positive approach to care by providing the following:

  • Knowledgeable clinical and program staff
  • Accessible pap testing for women with physical disabilities
  • Full-time onsite attendant care services
  • Height adjustable examination tables
  • Ceiling lifts
  • Fully accessible and barrier free facility

Youth Health Clinic

We provide client-centered care to help you meet your health needs. Vibrant Healthcare Alliance has an interdisciplinary team, which means that you can come to one place to get checked for an STI, to see a therapist, or be referred to another specialized service that we cannot provide directly.

Our free and confidential services include the following:

  • Birth control options
  • Birth control and prescriptions at reduced rates
  • Emergency contraception (Plan B)
  • Pregnancy options counselling
  • STI treatment and testing
  • HIV testing
  • Free condoms and lube

Complex Older Adults

Available to seniors 55+ and needing a family doctor or nurse practitioner.

Seniors Home Health Program (SHHP)

The Seniors Home Health Program provides primary health care services to frail and homebound seniors with complex needs, ages 55 and up. Our interdisciplinary team of providers includes physicians, nurse practitioner, registered nurse, occupational therapist, physiotherapist, chiropodist and registered dietitian. The goal of the Seniors Home Health Program is to provide client-centered care that improves seniors’ ability to live independently and stay out of hospital.

Foot Care

Our chiropody (foot) clinic provides maintenance of general foot health & managing unexpected foot ailments.

In order to assist clients in their overall health, we have a chiropody (foot) clinic for maintenance of general foot health and managing unexpected foot ailments. We provide assessments and treatment for the following: diabetic foot ulcers, warts, skin infections, fungal nails, heel pain, foot pain and other foot problems. Soft tissue surgeries are also available for a variety of reasons such as ingrown toenails. 

Dietitian

The dietitian works with individuals and groups to support healthy eating for a variety of dietary needs

The dietitian works with individuals and groups to support healthy eating for a variety of dietary needs including chronic diseases, heart health, healthy weight and other dietary requirements. Staff also help individuals learn how to improve their health and stay healthy.
A range of services are available through individual appointments and group programs. The dietitian provides counselling, case management and individual advocacy to clients.

Social Workers

Our social workers provide counselling, case management and advocacy to clients; we provide individual, couple and family counselling.

Our social workers help people with a range of concerns including housing, income, family and relationship issues, feelings of depression and anxiety, coping with illness, dealing with past or current abuse and drug and alcohol use. We listen to our clients, help them discover their own strengths, work with them to find suitable solutions to their problems and refer them to other services available in the community.A range of services are available through individual appointments and group programs. The dietitian provides counselling, case management and individual advocacy to clients.

Occupational Therapy

In home intervention and psychosocial support to help individuals develop or maintain the skills to manage daily living at home safely, independently and with confidence

The occupational therapist provides in home intervention and psychosocial support to help develop or maintain the skills to manage daily living at home safely, independently and with confidence. They provide advocacy and health promotion strategies for individuals, families, caregivers and the community.

Physiotherapists

Physiotherapists assess level of mobility, strength, endurance and other physical abilities. Treatment plans focus on promoting wellness, mobility, and function.
